Application Instructions
Step 1: Download the 2009-2010 Joint Application for Service Programs form. Complete
this form offline. After completing this form, proceed to the online portion of
the application submittal.
Step 2: You are now ready to create an online account and fill out a few more questions
about your program and agency. Once you have created an account, you can log on
and off the system and edit your saved information up until you decide to submit
your application.
Step 3: Complete your Agency information form. You will also be asked to upload the
following attachments: Recent board of directors list, 2008 Year-to-Date Agency
Budget or Financial Statement, Most current independent audit report, Proof of
organization's non-profit status (first-time applicants only). Please have these
available when you are completing your agency information. Please note: Agency information only needs
to be completed once per agency and can be re-used for each program application.
Step 4: Once your Agency information has been completed, complete a Program Information
form for each program for which you wish to request funding.
Step 5: You are now ready to review and submit your application. After you submit the
application, no further editing is possible. Print the confirmation page as
proof of your submission.
Step 6: Download the Application Certification Form. Fill out and sign this form as
directed.
