Step 6: Mail Certification
Download the Application Certification Form.
Fill out and sign this form as directed and mail to the address given on the form no later than two weeks after submitting the application. Include with the certification form any required attachments that you were not able to upload previously in electronic format.
Please Note: Submitted applications may not be considered without the signed certification form.
